Due to patient confidentiality, we are unable to discuss any aspect of a patient’s medical record with anybody other than the patient, without express consent, except for someone holding a valid Health and Welfare Power of Attorney, or the parent of somebody aged under 13.
If you would like to consent for someone else to be able to discuss your medical records with practice staff, please complete the form below and provide your photo ID and return this to the surgery.

Access to Online Services
Your GP record includes information like any conditions or allergies you have and any medicine you're taking.
Most patients will automatically be given access to more information added to their GP record from November 2022 onwards. This includes letters, test results and appointment notes.
You can get access to your GP records by completing the form below and returning it to the surgery with photographic ID (i.e. driving licence).

Adults who have parental responsibility for a child can ask to have access to their child’s record if the child is aged under 13 years old. We will need to see evidence of parental responsibility with a copy of the birth certificate and photographic identification. This access will stop on the child’s 13th birthday.

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access.
If you are both registered at the same GP surgery, you can manage health services for others in the NHS App or by logging in to the NHS website.
If you are registered at different GP surgeries, the surgery setting up access will give you login information for an app or website you can use.
To have proxy access to another patients medical records you will need to complete the below form and return this to the surgery with photographic identification of the patient, the surgery will also follow up this request with a call to the patient.
